Minimum cost-sharing is for TruHearing-branded Advanced model hearing aids. Maximum cost-sharing is for TruHearing-branded Premium model hearing aids. Premium models are available in rechargeable style options for an additional $50 per hearing aid. Costs associated with the following excluded items are not covered by the plan and are, therefore, the responsibility of the member: Optional hearing aid rechargeability, ear molds, hearing aid accessories, additional hearing aid fittings/evaluations, batteries for rechargeable hearing aids, additional batteries for non-rechargeable hearing aids, hearing aids that are not TruHearing-branded, and costs associated with loss and damage warranty claims. Routine hearing exam and hearing aid copayments are not subject to the out-of-pocket maximum.
