A Humana PPO plan is a Medicare Advantage plan that offers a broad network of health care providers to choose from for your healthcare needs. Plan members can also seek care from any Medicare-approved doctor, hospital or specialist outside of the plan network who accepts Humana’s plan terms and agrees to bill the plan.3
If you use a doctor or hospital that is outside of the plan’s network, your costs could be higher.
A Humana PPO plan does not require you to select a primary care physician (PCP) within the plan network. This type of plan also does not require you to get a referral from your PCP in order to see a specialist.
If you are enrolled in a Humana PPO plan, your monthly costs will vary depending on which medical facilities you visit and what medical services you need.
Humana PPO plans offer monthly premiums as low as $0 in some areas.1
Humana Medicare Advantage plans provide the same coverage benefits as Medicare Part A (hospital insurance) and Part B (medical insurance). Certain Humana Medicare Advantage plans offer additional benefits not covered by Original Medicare.

Humana PFFS plans have mid-range out-of-pocket costs. With this type of plan, you can visit almost any Medicare-approved doctor, as long as they accept Humana’s plan terms and agree to bill the plan.5
Humana Medicare Advantage Special Needs Plans (SNPs) are a type of Medicare Advantage plan available to beneficiaries who meet certain criteria. Humana offers SNPs to beneficiaries who are eligible for both Medicare and Medicaid, and beneficiaries who have certain qualifying medical conditions.
In addition to combining Medicare Part A and Part B benefits with prescription drug coverage, these plans are tailored to better meet the needs of the beneficiaries they serve. You must meet the specific conditions of the SNP to be eligible.6
