Members of a Humana Health Maintenance Organization (HMO) plan generally choose a primary care physician (PCP) from the plan’s network who will coordinate care and make referrals to see specialists or other providers.
You can change your primary care physician at any time, as long as you select another provider from the plan’s network.

Plan members can receive approved care outside of the plan’s network of providers, though you may have higher out-of-pocket costs than if you received in-network care.

Members have the option of selecting a primary care physician if they wish but do not need a referral to see a specialist.
