Members of a Humana Health Maintenance Organization (HMO) plan generally choose a primary care physician (PCP) from the plan’s network who will coordinate care and make referrals to see specialists or other providers.
You can change your primary care physician at any time, as long as you select another provider from the plan’s network.
A Humana HMO plan may be right for you if you prefer to have your care coordinated by a primary care physician and if you are looking for an affordable Medicare Advantage plan option.
A Humana PPO plan allows you to see any Medicare-approved provider in or out of the plan’s network who accepts Humana’s plan terms. This means that you can enjoy additional freedom and flexibility in making your health care decisions.
Plan members can receive approved care outside of the plan’s network of providers, though you may have higher out-of-pocket costs than if you received in-network care.
Monthly plan premiums and copayments for office visits may be as low as $0 in some areas.1
A Humana PPO plan may be right for you if you want additional flexibility in your care decisions, including seeking treatment outside of the plan network.
A Humana Private Fee-For-Service (PFFS) plan allows members to receive care from any Medicare-approved provider who has agreed to the payment and terms of the plan.
Members have the option of selecting a primary care physician if they wish but do not need a referral to see a specialist.
A Humana PFFS plan may be right for you if you’re looking for greater transparency and cost predictability with your health care expenses.
Humana also offers Medicare Advantage Special Needs Plans (SNPs) to anyone who meets specific eligibility requirements, lives in the service area of the plan and is enrolled in Medicare Part A and Part B.

Each Humana Medicare Advantage plan may also include full coverage of many annual preventive screenings at no additional cost to you.
Humana Medicare plans may also include memberships to Silver Sneakers or other fitness or wellness programs, access to mail-order pharmacies, and more.
