Humana offers three Medicare Advantage plan types:2
Your plan options may vary depending on your location.
This plan offers affordable copayments, has no annual deductible and has lower out-of-pocket costs. The Humana Gold Plus® HMO plan requires you to use in-network doctors.3
This plan offers affordable copayments, has an annual deductible and has mid-range out-of-pocket costs. If you are enrolled in this plan, you can visit any Medicare-approved doctor who accepts Humana’s plan terms and agrees to bill the plan.4
This plan offers affordable copayments, has an annual deductible and has mid-range out-of-pocket costs. If you are enrolled in this plan, you can visit almost any Medicare-approved doctor who accepts Humana’s plan terms and agrees to bill the plan.5
Also, you may qualify for a Humana Medicare Advantage Special Needs Plan if you have a qualifying medical condition, are eligible for both Medicare and Medicaid and/or if you live in a long-term care facility.
If you are enrolled in a Humana Medicare Advantage plan, your monthly costs will vary depending on the plan you are enrolled in, which medical facility you visit and what medical services you need.
Each Humana Medicare Advantage plan may have a different monthly premium. Your premium will depend on which plan you choose.
Keep in mind that Humana offers affordable plans that may fit your coverage needs and budget. You may be able to enroll in a plan with a $0 monthly premium.6
The medical providers and facilities you use may impact your monthly costs. Depending on the plan you choose, you may need to visit in-network providers.
For example, if you enroll in the Humana Gold Plus® HMO plan and use a doctor out of the plan’s network, your costs may be higher.
The medical services you need will impact your monthly costs.
For example, if you visit your doctor frequently and need specialty services, your costs may be higher because you need to pay the plan’s set copayments, coinsurance and/or deductibles for those services.
